Health at Homeâ„¢ , an LCS company, is seeking an experienced RN Case Manager to provide home health management in the Southbury and Redding, CT areas. The qualified candidate will be working out! of two upscale retirement communities; Pomperaug Woods www.PomperaugWoods.com and Meadowridge www.MeadowRidge.com .
Performs health care services for the client that reflects the agency's philosophy and standards of nursing practice.

Required Skills
Current, Valid CT RN license.
Must meet all requirements for applicable state regulatory agencies.
Ability to read, write, and communicate in English.
Requires effective interpersonal skills. Must have excellent organizational skills and problem solving abilities.
Must ! be a team player, flexible and able to handle multiple tasks. !
Strong customer service and interpersonal communication skills.
Required Experience
Minimum 2-3 years of work experience as a Registered Nurse in a home health, hospital or health care facility.
