The Home Care Aide Supervisor in our Southington, CT office will manage per diem Home Care Aides for our Central CT regio! n and comply with all regulatory requirements for training and patient care, and compliance with all policies and procedures.
Duties:
Maintains compliance with regulatory agencies; applies Agency policies and procedures resulting in deficiency free surveys
Provides support and guidance to care management teams concerning performance problem resolution
Resolves patient care problems collaboratively with internal/external customers
Anticipates customer needs and responds quickly, accurately, and pleasantly
Develops and revises home care aide new hire orientation programs with input from Care Management and Quality Management departments; develops competency testing for aides with Human Resources and Care Management departments
Participates in the development of annual in-service programs in association with the Care Management and Quality Management departments
Monitors and communicate regulatory and ac! creditation changes relative to the home care aide area. Funct! ions as a resource to others.
Insures appropriate resource utilization to meet client needs
Manages home care aide pool, providing guidance to scheduling function
Hires, trains, coaches, and evaluates staff
Determines staffing needs and coordinates hiring to meet service demands
Conducts home visits with staff to supervise and evaluate performance; identifies and follows through with development plan
Participates in preceptor programs and student education
Will actively participate in the performance improvement process known as H3W.
Requirements:
RN with valid CT license
Assoc Degree or diploma in nursing; BSN preferred
Minimum of one year as a supervisor of aides/CNAs in a clinical setting; two years of home care clinical nursing experience; additional year or more of nursing experience (3 years total nursing experience plus 1 year supervisory experience with aides).
