Job Description
The Senior Vice President of Care Management will be responsible for:
Providing the overall strategic and operational direction for the company’s care management priorities and capabilities in partnership with the senior management team
Developing innovative ways to improve care within bundled payment programs and the changing healthcare environment
Interacting well with the company’s diverse leadership team that includes physicians, statisticians, nurses, insurance executives, provider executives and analysts
Building the care management team, including lead and staff Patient Navigators and care coordinators who will be onsite at our company's partners
Balancing strategi! c thinking and protocol development with infrastructure development operational management, execution, customer interactions, performance tracking, new customer acquisition, etc.
Building infrastructure and safeguards that ensure the delivery of high quality care and compliance a myriad of regulatory and privacy requirements
Developing and training staff to deliver care and working with our company's partners in accordance with the provider's specific care model and protocols
Keeping abreast of new tools and techniques that will enable the company to continually differentiate based on sophisticated and targeted care management strategies
Developing innovative ways to improve care through the bundled payment model
Working collaboratively with the company's team and partners to develop new and interesting ways to improve care and outcomes
Required Skills
10+ years of experience in clinical care management activiti! es; 4+ years of management experience leading clinical care management ! teams and activities
Master of Science or PhD in Nursing or related clinical field
RN or NP license
Current clinical experience in the care of seriously ill older adults
Familiarity and experience with nationally recognized transitions of care models ( Naylor , Coleman, Jack, et al)
Familiarity and experience with care and case management workflow systems, functional assessment and risk stratification tools
Familiarity with relevant care protocols and practices as delivered by acute care, home care, and long term care providers
Ability to collaborate with and motivate clinicians, cross-functional partners around key clinical initiatives and drive for results and effective execution
Excellent communication skills
