Hospice RN Home Care Case Manager needed for our Central CT office: 40 hours/week, Mon - Fri with some weekend rotation. Responsible for delivering high quality, compassionate hospice, oncology and palliative care for home hospice patients. Will coordinate all client care services; develop plans of care including setting up and coordinating a variety of Agency ancillary services. Works with LCSW, pastoral counselors, and bereavement coordinator to assist patient/ families through transition.
Will work out of our Southington, CT office, and will cover all towns in Central CT territory.
Vvalid CT RN license.
Must have at least one full year of licensed RN experience in hospice/oncology in home care, hospital, dedicated hospice or other facility as a full or part-time RN working with adult/geriatric patients. PREFER: Minimum 1 year previous recent home care experience with hospice patients (admissions/OASIS and visits with hospice/palliative and med/surg ! treatments).
Prior experience with home care software/lapt! op programs preferred; good computer skills are required.
