In this role the candidate will be working onsite with various physician groups in the greater Hartford area.POSITION SUMM! ARY
This position focuses on effective collaboration between our Case Manager and the physician groups associated with the ProHealth Accountable Care Organization. A requirement of this position is for the Case Manager to work in an integrated environment, in close proximity to each identified physician practice, ProHealth staff and ActiveHealth sites. The Case Manager will potentially work from various health care sites in managing and following members who are identified for focus follow-up. This position requires disciplined time management and organizational skills, along with the ability to manage continually changing priorities as needed.
The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license.
EDUCATION
The highest level of education desired for c! andidates in this position is a Bachelor's degree or equivalent experience.
