We're looking for an experienced RN Case Manager for home care case management and OASIS admissions in the western Waterbu! ry/Watertown/ Tribury area (Southbury/Middlebury/Woodbury), CT area:
Full time, 40 hours per week (Mon-Fri), with full benefits, on our downtown Waterbury Med/Surg team. Ensure delivery of outcome-based, high quality, cost effective home care to individuals in their homes. Will coordinate all client care services; develop plans of care including setting up and coordinating a variety of Agency ancillary services, setting and monitoring short and long term goals, and discharge planning.
Requires: Valid CT RN license. PREFER: Minimum 1-2 years previous recent home care med/surg experience (OASIS admissions of Medicare/ Medicaid adult/geriatric patients) treating acute and chronic med/surg issues: Diabetes care, wound care, COPD, post surgical, CHF, etc.).
Will consider training RN with at least 2-3 years recent experience in acute care med/surg hospital or acute care facility working with adult/geriatric patients. ICU/CCU experience preferred.! Must have sharp assessment skills.
Prior experience ! with home care software/laptop programs preferred; good computer skills are required; previous experience with medical charting on computers required.
