The RN Case Manager provides the necessary and appropriate direct service; consultation and coordination to facilitate expected cli! nical outcomes for home care patients and their families. The RN coordinates the plan of care in accordance with the attending physicians orders and Agency policies, procedures and practices. Delegates appropriate patient care responsibilities to LPN and Home Health Aides, and supervises their performance. Teaches and orients LPN, Home Health Aide, patient and family members in the care of the patient. Responsible for accurate, timely, and proficient clinical and reimbursement documentation. Performs related duties in support of the Nursing Department and the Agency.

ESSENTIAL FUNCTIONS

Must be able to perform essential functions with or without reasonable accommodation.

Strength/Mobility: Ability to walk and stand. Able to sit for prolonged periods. Able to move about in confined spaces and in and out of automobiles. Able to travel between floors. Able to stand for prolonged periods and to remain in uncomfortable positions for long periods! as in providing direct patient care procedures. Able to push,! pull, and lift objects more than 25 lbs.

Manual Dexterity/Coordination: Frequent walking, bending, stooping involved in patient care activities. Able to perform difficult manipulative skills such as preparing injections, calibrating instruments, positioning patients etc. Able to perform tasks which require hand-eye coordination such as keyboard skills, giving injections, and arm-hand steadiness such as taking blood pressures and calibrations.

Visual/Auditory Discrimination: Able to see objects far away as in driving. Able to see objects closely as in reading thermometers, patient assessment observations, and in reading computer screen, etc. Able to discriminate colors as in traffic lights. Able to distinguish sounds as in auscultation and to hear normal sounds within some background noise as in individual conversation, answering phone, etc.

ENVIRONMENTAL CONDITIONS

Occupational Exposure Risk Category A is described as: Exposure to blood, body tissue or fluids working with patients with a wide range of medical problems. Exposure to other hazardous materials such as chemicals, in the treatment of patients. Exposure to sudden fluctuations in temperature and to seasonal conditions in outside weather in the course of traveling to and from patient homes. Exposure to dust and pets in patient homes. Average office environment with normal equipment noise and use of computers and other modern office equipment and technology.

POSITION RESPONSIBILITIES

Branch specific.

QUALIFICATIONS:
Education & Training: Graduate of a National League of Nursing accredited program with a diploma or Associates Degree in Nursing. Bachelor of Science Degree in Nursing preferred.

License/Certification: Current Connecticut RN license, or eligible for RN license in the State of Connecticut, or current temporary RN permit.

Experience: Medical/S! urgical experience or specialty area, if applicable, preferred. Experie! nce in Home Health Care or Public Health Nursing preferred.

Skills: Basic clinical nursing skills. Experience in physical and psychosocial assessment. Excellent verbal and written communication skills. Strong organizational skills. Excellent interpersonal skills; ability to effectively interact with patients, families, physicians and co-workers. Ability to deal effectively with stress and ability to exercise independent judgment.

Other: Valid motor vehicle operators license, reliable personal automobile during business hours, and active automobile insurance coverage in accordance with Agency policy.

OTHER JOB REQUIREMENTS:
Must be available to rotate weekends, holidays, and for after hour coverage, as required, to meet the needs of the Agency and patient care needs.
