DUTIES:
**The salary and grade of this position will be determined by the Nurse Professional Standards Board upon appointment**

GEC is recruiting for a newly funded RN position for the Caregiver Program.

This individual will be responsible for all in-home initial and follow-up assessments for those enrolled in the caregiver program.

Because of the home based focus of this position the plan is to cross train this individual to HBPC (Home Based Primary Care). The Caregiver Nurse must be willing to visit Veteran homes where there could be animals such as family pets. This is a Monday-Friday position wit! h flexible hours of 7:30AM-4PM or 8AM-4:30PM.

The Caregiver Program Coordinator is Bonnie Cecarelli, Social Work but this position will have oversight by Linda Accordino, Director/GEC since the position is aligned under GEC.

Use of a GSA vehicle is authorized. Travel is anywhere in the state as long as the caregiver is eligible for the program.

Responsibilities:
o

Perform all initial assessments, follow-up

and care coordination in home environment for Veterans enrolled in caregiver program

o

Provide ongoing, comprehensive evaluation of a caseload of patients to meet their needs in the home

o

Confer with the Caregiver interdisciplinary team to evaluate changes to ensure quality care

o

Identify problems and areas of concern and reports these concerns to patientÂ's primary care provider
o

Serve as a resource and advocate in the protection of! patient/family abd staff rights

o

Take action to avert or manage situations in a manner that meets the needs of the patients in a safe and therapeutic manner.

QUALIFICATIONS REQUIRED:
Qualifications and requirements include:

RN; BSN preferred

·

Mental health experience required ; expertise in PTSD preferred

·

Previous home care experience desirable

·

Unrestricted driverÂ's license required

·

Good

interpersonal, communication and organizational skills

·

Exceptional documentation skills

·

Willingness to cross-train to HBPC

·

Ability to travel to homes of patients enrolled in the Caregiver Program; home visits could provide exposure to animals (pets and non-family pets); inclement weather; stressful situations that exist in the home

·

Work as a member of an ! interdisciplinary team providing care to Veterans in the Caregiver program

·

Ability to be self-directed in prioritizing and directing patient care and organizing visit schedules in order to insure high quality, cost effective care

·

Ability to shift priorities based on patient, environmental and program needs

·

Knowledge of homecare standards preferred

·

Computer skills required

Ability to drive for extended periods of time and able to lift objects such as laptop cases, nurse bag and other home assessment supplies in and out of vehicles and into homes as necessary
