The Hospital of Central Connecticut has two full time positions available for a Clinical RN Case Manager at our New Britain campus on the day and evening shifts. Hours are Monday thru Friday, 8:00am-4:30pm OR 3:00pm-11:30pm. Occasional on call responsibilities on weekends and holidays. The Care Coordinator is responsible for facilitating the patient’s hospitalization from preadmission through discharge. The Care Coordinator coordinates with physicians, nurses, social workers, and other health team members to expedite medically appropriate, cost effective care. The Care Coordinator applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. S/He will advise the healthcare team and provide leadership as needed.

Requirements This position requires a Bachelor's degree in Nursing or a related field..

Minimum requirements include ? Current RN license in the state of Connecticut. ? Must have a minimum of 4 years of experience in utilization review or case management OR 5 years of recent acute care experience. ? Strong written and verbal communication skills. ? Strong interpersonal, managerial and educational skills. ? Knowledge of clinical and fiscal interactions across the continuum of care. ? Strong working knowledge of Medicare and reimbursement plans. ? In-depth skill and medical knowledge to manage patient care and maximize timing of sequence of activities. ? Ability to conduct comprehensive patient assessment and analyze data to formulate an appropriate plan for patient management. ? Preferred Qualifications ? Case management certification or eligibility for certification is preferred.
