The Nurse Navigator assists cancer patients and their families from screening through to! survivorship in navigating through the healthcare system so that they can focus on healing. The navigator plays a key role in teaching patients and family about treatments including; common side effects, treatment schedule, and medication information and their treatment course. The Nurse Navigators acts as the liaison to community resources and assists the patient in accessing those services. Minimum Qualifications: BSN required with current CT Licensure. Masters preferred or enrolled in Master's study in health care related field. 5+ years experience preferably with oncology patients. Excellent and proven symptom management and cancer care skill set. Strong and effective communication skills both verbal and written are imperative. Proven ability to coordinate and facilitate timely needs of a large patient load. Proven ability to coordinate and facilitate care amongst multiple disciplines. Essential Functions: *Demonstrates the knowledge and skills necessary to provide sup! port and education to cancer patients throughout the trajector! y of care to achieve desired outcomes for the patient with consideration of age, educational readiness and cultural differences. *Maintains an interdisciplinary approach to patient care, support and resource referrals. Participates in the Multidisciplinary Tumor Boards and Multidisciplinary Rounds as appropriate. *Coordinates needed services across all product or service lines while ensuring proper access to these services. Evaluates and recognizes changes in patient's physical and psychosocial status and communicates changes to proper care givers. Links patient, caregivers and families with appropriate follow-up and support services. *Establishes effective communication with healthcare professionals to facilitate an interdisciplinary approach to the total management of the cancer patient. Identifies and addresses barriers that may arise. *Maintains a current knowledge of available community resources and utilizes this knowledge to assist the patient in accessing these serv! ices. *Assists the Department of Oncology in strategic and quality initiatives including data collection, analysis and reporting.
