As a Hospice Registered Nur! se Case Manager, you will follow your patients over the course of their end of life journey. This allows you to utilize your clinical skills while you develop meaningful relationships with your clients, their families, and with other members of our extraordinary team.

Job Qualifications:
The ideal candidate will have 1 to 2 years of experience in a Hospice, Long Term Care, or Oncology setting. We will also train a nurse with strong assessment skills and two years of nursing experience with an interest in caring for hospice patients. Must be willing to travel in their local area. Additionally, the ideal candidate will possess exceptional clinical s! kills, a devotion to patient centered care, and a warm engaging personal style.
Additional Requirements:
A reliable automobile (you will be reimbursed for mileage)
Current state driver's license, Current car insurance
Valid professional RN license
Computer proficiency
