Description: National Health Care Associates, Inc. is currently seeking to employ MDS Director right now, this occupation will be placed in Connecticut. Regency House Nursing and Rehabilitation Center is currently conducting a search for an MDS Director to lead the Reimbursement Team! To be successful in this role you will need at least 3-5 years of proven success and recent 3.0 expereince. As the MDS Director you will need to be the expert in MDS with Medicare experience as well as manage others in the process.

MDS Coordinator/Director

General Statement of Job Function : Responsible for overseeing comprehensive resident care plans through clinical assessment, r! eview of resident's medical history, personal interviews and c! ompletion of MDS. Responsible for coordinating interdisciplinary resident care conferences with Social Services staff.

Minimum Qualifications:
1. Ability to read, write and comprehend English; ability to follow oral and written instruction.

2. Interest in the needs of the aged and chronically ill, with the ability to work with the elderly.

3. Ability to work with supervisors, co-workers and facility staff in the performance of duties.

4. Ability to work hours as scheduled based on the requirements of the position assignments.

5. Licensed Registered Nurse in the State of employment.

6. Has working knowledge of MDS and PPS process

7. Knowledge of Submission Process and Interpretation of Validation Reports

8. MDS certified preferred.

PRIMARY FUNCTIONS :
Coordinates and completes the MDS with appropriate CAAs and trigger worksheets; determines assessment reference dates.
Reviews resident's medical history.

Interviews appropriate unit staff.

Reviews resident care plans using information available and resident care plan library during MDS completion window.

Daily review of rehab software for unscheduled assessments.

OTHER DUTIES:
Updates resident care plan library as necessary.

Monitors follow-through of resident care plans with Unit Managers, report non-compliance to DNS.

Audits Resident's Medical Record for Q.A. for areas with MDS relevance.

Ensures correct/timely computer input.

Remains current/seeks appropriate in-servicing.

Presents a neat appearance with a pleasant speaking voice and personality. Must be punctual.

Works with a minimum of supervision.

Discusses any problems, needs or concerns which arise with the Administrator or DNS/ADNS.

Attends in-service educational classes and on-the-jo! b training programs as directed.

Creates and maintains an atmo! sphere of warmth, personal interest, and a positive image throughout the facility.

Treats people with respect; works with integrity and ethically; upholds organizational values and mission statement; supports affirmative action and respects diversity.

Understands and upholds Residents’ Bill of Rights and holds all resident information confidential and follows HIPAA guidelines.

Understands responsibilities concerning infection control, safety, accident and fire prevention.
