The Care Coordinator is responsible for facilitating the patient’s hospitalization from ! preadmission through discharge. The Care Coordinator coordinates with physicians, nurses, social workers, and other health team members to expedite medically appropriate, cost effective care. The Care Coordinator applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. S/He will advise the healthcare team and provide leadership as needed.

Educational Requirements:
Bachelor’s Degree in Nursing or a related field.

Minimum Requirements:
Current RN license in the state of Connecticut.

Must have a minimum of 4 years experience in utilization review or case management.

Strong written and verbal communication skills.

Strong interpersonal, managerial and educational skills.

Knowledge of clinical and fiscal interactions across the continuum of care.

Strong working knowledge of Medicare and reimbursement plans.

In-depth skill and medic! al knowledge to manage patient care and maximize timing of seq! uence of activities.

Ability to conduct comprehensive patient assessment and analyze data to formulate an appropriate plan for patient management.

Preferred Qualifications:
Case management certification or eligibility for certification is preferred.

Master's Degree preferred.

Hours:
16 hours every Saturday and Sunday; 8:00am to 4:30pm.
