The nurse practitioner provides direct primary health care services to individuals and families. Provides direct primary health care services to adolescent, adult and geriatric patients. Utilizes clinical skills of assessment, planning, implementation, and evaluation.
Documents according to Health Center standards. Provides for the teaching needs of adolescent, adult and geriatric patients, their families, and other health care providers to ensure patient education needs are met. Knowledge, skills and dexterity to perform advanced physical assessment and provide direct primary care service. Manual dexterity to administer medications and complete diagnostic evaluation in Ambulatory setting.
Ability to work autonomously and collaboratively with clinical and administrative supervisors Connecticut RN and APRN license. Current Certification for Family Nurse Practitioner by National Organization Minimum 2 years A.P.R.N. experience. Master’s Degree of Science, Graduate of an approved program for Family Nurse Practitioner
