Location: Norwalk Connecticut

Description: Norwalk Hospital is at present looking to employ APRN, Palliative Care Program right now, this job will be placed in Connecticut. The primary function of the Palliative Care APRN is to provide efficient and high quality medical care to inpatients requiring palliative care assessment and plan of ! care. Under the direction of the Director of Operations, DOM, the Palliative Care APRN :
Coordinates the plan of care with physicians, case managers, social workers, nurses and outside agencies as appropriate
Conducts clinical practice and demonstrates decision-making based on expert knowledge in the assessment, diagnosis, planning, and management of complex human responses to life-limiting, serious illness
Assists the Palliative Care team in attending to the palliative care needs of the patient/family to promote a comprehensive plan that incorporates psychosocial, spiritual, and biomedical care
Orders appropriate diagnostic tests based on differential diagnosis; implements patient population specific interventions based on individual patients needs
Communicates and collaborates with the primary and/or alternate supervising physicians regarding clinical findings and patient care issues in a timely manner

Job Requirements:
The follo! wing are required:
Masters Degree in Nursing with Nurs! e Practitioner degree
Currently licensed or eligible for licensure within the State of CT as a Nurse Practitioner
CT State Controlled Substance License II through V
Federal DEA Certificate II through V
BLS Certification
Superior communication skills
Demonstrates independent problem solving, decision making, organizational and prioritization skills
Ability to effectively manage time, prioritize work and multi-task

The following are preferred:
National certification as Advance Certified Hospice and Palliative Care Nurse (ACHPN) preferred
3 or more years of previous clinical experience in nursing strongly preferred
1 or more years of experience in palliative care strongly preferred
