The Northeast Medical Group/YNH Geriatric Services is recruiting for a full time, experienced physician to join our staff in caring for patients at affiliated nursing facilities and retirement communities in the greater ! New Haven area. Current CT physician license, state and federal drug registration certificates mandatory. ABIM certification required, added qualifications in Geriatrics preferred. The Practice serves affiliated nursing homes to provide attending physician care for both long and short term patients. Providers have their own patient panel and are responsible for admission, routine monthly, problem focused and discharge visits for nursing home patients. The Practice also provides primary care to older patients in our affiliated retirement communities and outpatient settings. All members of the Practice participate in teaching initiatives for medical residents, PA students and Geriatric fellows. All members of the Practice agree to follow proper documentation and billing procedures utilizing our electronic medical record and billing systems. Weeknight, weekend and holiday on-call participation is required. We are dedicated to providing the finest medical care available to the ! patients and facilities we serve.
