Liberation Programs is seeking a part-time LPN for an opening in our Stamford location Monday - Friday from 6:00 a.m. - 10:00 a.m. The part-time LPN helps carry out the agency mission and! functions as part of the medical services unit. Specifically, the LPN will: 1) provide back-up for the methadone dispensing nurse, 2) assist in triage and support of medical care coordination; and 3) monitor self-administration of medication, handling and storage for residential clients.

DUTIES

Outpatient Clinic

  • Provide "back-up" and relief for the methadone dispensing nurse
  • Dose via Avatar and hand dose in emergency situations
  • Calibrate equipment and close out for the day
  • Document in Avatar as needed
  • Maintain information in an electronic medical records database (AWARDS) as needed to provide medical care coordination
  • Provide patient education on medications, side effects, medical conditions.
  • Lead "wellness" efforts for clients
  • Transcribe physician orders when necessary
  • Run reports as needed
  • Maintain clean equipment and dispensing area
Residential Prog! rams
  • Provide oversight for self-administration of! medication in residential including receipt of medication, storage of medication, labeling and disposal of medication.
  • Monitors and documents observation of self administration.
  • Care coordination between residential and outpatient medical services.
  • Fills orders for medications with pharmacy
  • Provide patient education on medications, side effects, medical conditions
  • Provide training to residential staff in collaboration with Medical Director on self administration of medication as required
  • Triage requests for "sick visits" to physician.
  • Deliver exceptional customer service in both programs
QUALIFICATIONS

Current CT Licensed Practical Nursing license (LPN).
  • Three to five years of direct patient care in hospital, primary care, psychiatric facility or home care. Experience in substance abuse/methadone dispensing preferred.
  • Knowledge of the principles of addiction treatment a! nd practical experience in the field of addiction treatment is preferred.
  • Computer literate. Demonstrated ability with Microsoft Outlook. Familiarity with methadone dispensing software preferred.
  • Strong oral and written communication skills along with the interpersonal skills to work effectively at all levels of the organization and its stakeholders. Outpatient Clinic
